<h3>Step 1 &#8211; Make sure you are eligible for Healthy Way LA</h3>

<td>Before you apply for Healthy Way LA, please look below to see if you are eligible. You may be eligible if:</p>
<ul>
<li>You are a Los Angeles County resident.</li>
<li>You are between 19 and 64 years of age.</li>
<li>You are not pregnant.</li>
<li>You have no health coverage.</li>
<li>You have been a legal resident for at least 5 years or you are a United States citizen.</li>
<li>You meet monthly income limits.</li>

<h3>Step 2 &#8211; Gather the required documents to apply for Healthy Way LA</h3>

<td>If you meet the initial <a title="Healthy Way LA Eligibility" href="http://healthywayla.org/"><span style="text-decoration: underline;">Healthy Way LA eligibility</span></a> listed above, your next step is to apply for services. You need to make sure you have all of the following documents required by Healthy Way LA . These documents include:</p>
<h4>For Citizenship / Residency</h4>
<ul>
<li>U.S. Passport, or</li>
<li>Certificate of Naturalization, or</li>
<li>Certificate of Citizenship, or</li>
<li>Permanent Resident Card (Green Card)</li>
</ul>
<p><strong>NOTE:</strong> If you do not have a U.S. Passport, Certificate of Naturalization, Certificate of Citizenship, or Permanent Resident Card, bring one document from each category:<br />
1) U.S. Birth Certificate or Military Service Form that shows place of birth</p>
<p><strong>- And -</strong></p>
<p>2) Driver’s License, or School Identification with Photo, or U.S. Military Identification Card</p>
<h4>For Income Verification</h4>
<ul>
<li>Recent paycheck stub, or</li>
<li>Copy of last year’s federal income tax return, or</li>
<li>Signed letter from your employer that shows the gross amount paid, how often you get paid, and the date of paycheck being reported.</li>
</ul>
<p>Note: If you appear to be eligible for Medi-Cal or Healthy Families, you must cooperate with those programs before applying for HWLA by completing a Medi-Cal or Healthy Families application.</td>
